• Choose between 2 LOA forms:
  • Individual LOA:Letter of Agency for Preferred Carrier Change
    • Use this form for individual port requests (with no repeats subsequently).
    • Customer must complete and sign this form each time.
  • Blanket LOA:Letter of Agency for Preferred Carrier Change Blanket Approval
    • Customer completes and signs this form one time and anytime there is a subsequent port request within a 12-month period.
    • Attach a copy of the previously signed blanket LOA to a new Billing Telephone Number (BTN) Selection Form for each port order.
      • Either add Telephone Numbers (TNs) on the BTN Selection Form or attach a separate document to the BTN Selection Form with the TN and check the box on the BTN Selection Form that states: "separate page for additional TN."
        • The BTN Selection Form is the second page of the Letter of Agency for Preferred Carrier Change Blanket Approval.
          
          • A new Blanket Approval must be signed each year.
