Account Members - Transfer service from a deceased Account Owner

Transfer service to another Verizon account.


If you're an Account Member on a Verizon account for which the Account Owner has passed away, use this form* to notify us and transfer the account's lines to a different Verizon account.

Once we're notified of the Account Owner's death, the lines on the account must be transferred to a new Verizon account within 30 days. Lines that aren't transferred will be disconnected.

Note: If lines are being transferred to more than one Verizon account, submit a form for each account with the deceased's paperwork.

*If you're the executor of the estate, you must use the Executor Transfer Form and not this form.

To notify us of your transfer request, complete the transfer service form. The death certificate documentation is required. All fields are required.

Please attach a copy of the Account Owner's death certificate. File types accepted are .jpg, .pdf and .png, and the maximum file size is 10Mb.


Note: Before attaching, please redact the deceased's private information to protect the deceased's identity (e.g., Social Security number, veteran's discharge or claim number, parental names, birthplaces, etc.).
