Transfer service to another Verizon account.


Account Managers:

  • You can use this form only if we've asked you to upload documentation for a disconnect service request.
  • To transfer a line of service to another account, please follow the Transfer your Service process in My Verizon.

Account Members:

  • Please complete this form to transfer service to another Verizon account.
  • You can also use this form If we've asked you to upload documentation for a transfer service request or a disconnect service request.

Important: If you're the Estate Executor, you must use the Executor Form and not this form.

Please complete the fields below. All fields are required. The death certificate documentation is required. Account Members - If lines are being transferred to more than one Verizon account, complete a form for each account. You must attach the death certificate documentation to each form.

Please attach a copy of the Account Owner's death certificate. File types accepted are .jpg, .pdf and .png. Maximum file size is 10Mb.


Important: Before attaching, please redact any private information to protect the deceased's identity (e.g., Social Security number, veteran's discharge or claim number, parental names, birthplaces, etc.).
