Executors - Transfer service when an Account Owner is deceased

Transfer service to another Verizon account.


If you're the Executor of the estate of a loved one who has passed away, complete this form to notify Verizon of the Account Owner's death and transfer the lines to a different Verizon account.

You can also use this form if we asked you to upload documentation for a transfer service request or a disconnect service request.

Note: If lines are being transferred to more than one Verizon account, complete a form for each account. Include the Letters Testamentary document for each form.

Please attach a copy of the Letters Testamentary document that certifies you as the executor. File types accepted are .jpg, .pdf and .png. Maximum file size is 10Mb.

Important: Before attaching, redact any private information to protect the deceased's identity (e.g., Social Security number, veteran's discharge or claim number, parental names, birthplaces, etc.).
