Executor's form - Transfer service from a deceased Account Owner

Transfer service to another Verizon account.


If you're the executor of the estate of a loved one who has passed away, complete this form to notify Verizon of the Account Owner's death and transfer the lines to a different Verizon account.

Once we're notified of the Account Owner's death, the lines on the account must be transferred to a new Verizon account within 30 days. Lines that are not transferred will be disconnected.

Note: If lines are being transferred to more than one Verizon account, submit a form for each account. Include the Letters Testamentary document for each form.

Please attach a copy of the Letters Testamentary document that certifies you as the executor. File types accepted are .jpg, .pdf and .png, and the maximum file size is 10Mb.

Important: Before attaching, redact the deceased's private information to protect the deceased's identity (e.g., Social Security number, veteran's discharge or claim number, parental names, birthplaces, etc.).
